About this resource: The Transportation Call Center is a one-stop center with consolidated transportation resources and services. The Call Center helps with ride authorizations, public transit, Dane County Department of Human Services transportation programs, and other services.

Dane County Department of Human Services // Retired Senior and Volunteer Program (RSVP)

Phone: (608) 441-7898 (If you are in Madison)

About this resource: RSVP provides scheduled individualized rides to medical appointments and at times other destinations. This service is door-to-door and drivers will assist passenger’s in getting to the correct location within the clinic or hospital. Rides are provided in the volunteer’s own cars and are usually not accessible. Service area is all of Dane County.

Who can use this resource? Adults 60 and older and people with disabilities who live in their own homes or apartments. Individuals who receive MA rides through Common Carrier Medical Assistance Transportation are not eligible. Individuals enrolled in Long-Term Care through Family Care, Family Care Partnership, and IRIS are not eligible.

**Donations are accepted but no one is denied service based on inability to pay.

MTM: Non-Emergency Medical Transportation

About this resource: MTM offers Non-Emergency medical transportation to individuals enrolled in specific programs.

Who can use this resource?  Individuals enrolled in these programs:

  • Wisconsin Medicaid (including IRIS)

  • The BadgerCare Plus Standard Plan

  • The BadgerCare Plus Benchmark Plan

  • The BadgerCare Plus Express Enrollment for Pregnant Women

  • Tuberculosis-Related Services-Only Benefit

  •  Family Planning Only Services

Next Steps:

  • Routine ride requests can be scheduled Monday through Friday 7:00 AM- 6:00 PM by calling 866-907-1493

  • You will need the following information when you call:

    • Name, home address, and phone number

    • Forward Health ID number (10-diit number listed on your ForwardHealth card)

    • Street address and phone number where you want to be picked up

    • Name, phone number, address, and ZIP code of the health care provider you are seeing

    • Date and start time of your appointment

    • End time of your appointment (if you know it)

    • Any special needs, including if you need someone to ride with you.

    • General reason for the appointment (check-up, eye appointment, etc.)

If patient has iCare (Indpendent Health Care Plan), navigator needs to investigate what services iCare offers for medical transportation.

Insurance Needed - upon calling they will ask if you are a client or a care assistant. You should do the option for a facility or care manager (Option 2)

What you need when calling:

  • Forward Health Member ID

  • Full Name

  • Birthday

  • Appointment Times

  • Pick Up Address & Appointment Location

  • Eligibility Address (may be current address)

Transportation will be billed through Forward Health Insurance. If the client is eligible, then it will just be billed through insurance. Client needs to be eligible for coverage. 

At least 2 day notice is required. Appointments can be scheduled up to 30 days in advance.
