About this resource: Due to changes in funding, NHCC will no longer be offering a gas card option for our transportation assistance program. They will continue to open applications for bus passes on a quarterly basis. The application will be available to fill out in person or online. Applicants can receive one pass per year and priority is given to new applicants. Assistance is provided in the form of either one bus pass or one gas card.
Who can use this resource? Funding for this program is limited and individuals may only apply once per cycle. Priority is given to those who haven’t yet received assistance from this program previously and meet other requirements.

Please note that NHCC DOES NOT have bus passes on hand to hand out outside of application windows.

After applying, applications are reviewed and then individuals are contacted if they will be receiving a pass or not.

Passes are then available for pick-up at the office or mailed to recipients via USPS. After passes are distributed, we CANNOT issue a second pass. This means if there are issues with the mail, we will not be sending another one.
Madison Metro // Low Income Pass

About this resource: Metro 31-Day passes are available to eligible low-income riders for $28. Passes are available on a first-come, first-serve basis on the 1st and 15th of every month. Supplies are limited, so we encourage you to buy your passes on or as close to these dates as possible.
Who can use this resource? Individuals whose income is at or below 150% of national poverty guideline

Individuals must fill out a self-certification form that income is at or below 150% of national poverty guidelines. Forms can be completed at the time of purchase and no additional paperwork is required. A six-month low-income eligibility card will be issued.

If you already have a low-income pass eligibility card, you may show that to prove you are eligible at time of purchase.

You may also show a valid QUEST card to prove eligibility at time of purchase.

Joining Forces for Families (JFF)

About this resource: JFF has a very limited number of 2-ride tickets to distribute each month. The bus tickets are usually gone halfway through a given month.

UW Health Derzon Fund
About this resource: The Derzon Fund is used to help UW Health patients with expenses such as transportation, medications, lodging, meals, and other needs that may arise.
Who can use this resource? All UW Health patients BUT this resource should only be used if all other options have been exhausted.
** At Wingra, some patients are considered Access patients, and some are considered UW Health patient (Double check with front desk staff to find out if a patient is a UW Health patient)
