About this resource: Neighborhood House's Transportation Assistance Application Opens Tues 11/15 at 12 Noon. This program provides free a 31-day bus pass or a $50 fuel gift card to individuals in need of transportation assistance for work, school, medical visits, food access, and childcare. Assistance is provided in the form of either one bus pass or one gas card. The gas card will be to a gas station and the bus pass will be  

 

Who can use this resource? Funding for this program is limited and individuals may only apply once per cycle. Priority is given to those who haven’t yet received assistance from this program previously and meet other requirements.

Next Steps:

  • The application will be open until Friday November 18 at noon, or until we reach capacity - whichever comes first. Individuals can apply for either a bus pass or gas card and must supply all information on the application. More information will be posted at: neighborhoodhousemadison.org/transportation-assistance

  • Fill out the online application at neighborhoodhousemadison.org/transportation-assistance

  • Recipients will be notified and the $50 gas cards and 31-day bus passes will be distributed in early December.

  • Gas cards and bus passes can be picked up at Neighborhood House when they are available or will be mailed to the address provided in the application once approved.

  • 2022 Transportation Assistance Dates:

    • Cycle One: Application due February 2022, distribution begins in early March

    • Cycle Two: CLOSED  distribution begins in early June

    • Cycle Three: CLOSED distribution begins in early September

    • Cycle Four: Application opens November 15, 2022, distribution begins in early December

Madison Metro // Low Income Pass

About this resource: Metro 31-Day passes are available to eligible low-income riders for $28. Passes are available on a first-come, first-serve basis on the 1st and 15th of every month. Supplies are limited, so we encourage you to buy your passes on or as close to these dates as possible.

 

Who can use this resource? Individuals whose income is at or below 150% of national poverty guideline

  • Individuals must fill out a self-certification form that income is at or below 150% of national poverty guidelines. Forms can be completed at the time of purchase and no additional paperwork is required. A six-month low-income eligibility card will be issued.

    • If you already have a low-income pass eligibility card, you may show that to prove you are eligible at time of purchase.

    • You may also show a valid QUEST card to prove eligibility at time of purchase.

Joining Forces for Families (JFF)

About this resource: JFF has a very limited number of 2-ride tickets to distribute each month. The bus tickets are usually gone halfway through a given month.

Who can use this resource? Anyone!

  • Call or go to your nearest JFF office to see if they have bus tickets available.

    • Navigators can check nearest location and call before sending patient to a JFF office since ticket availability is so low.

UW Health Derzon Fund

About this resource: The Derzon Fund is used to help UW Health patients with expenses such as transportation, medications, lodging, meals, and other needs that may arise.

 

Who can use this resource? All UW Health patients BUT this resource should only be used if all other options have been exhausted.

** At Wingra, some patients are considered Access patients, and some are considered UW Health patient (Double check with front desk staff to find out if a patient is a UW Health patient)
