Public Health (Nurse-Family Partnership and Prenatal Care Coordination)
Public Health’s two free perinatal programs, Nurse-Family Partnership (NFP) and Prenatal Care Coordination (PNCC) have a new online referral form! The online referral forms in English and Spanish are available at the bottom of their pregnancy page.
Additionally, attached and below is a flyer with information about a virtual event for individuals that are pregnant and might be eligible for NFP or PNCC on Thursday, 11/18 from 6pm-7pm. The first 25 moms-to-be that sign up and attend the virtual event, will receive a $25 gift card.

Call the pregnancy helpline to request a baby care package after you give birth to your baby.

Safe Sleep

Provides a Pack ‘n Play portable crib, fitted crib sheet, and Halo SleepSack wearable blanket for babies up to the age of 15 months.

Parents should first fill out a request form- Call helpline to have it mailed to you.

When your baby is born, call the helpline to request delivery (may take 1-2 weeks).

Nurse Family Partnership Program // Public Health Madison & Dane County

About this resource: Individuals enrolled in this program get access to a free Nurse-Family Partnership personal nurse, who will help you get the support, advice, and information you need during your pregnancy and when your baby arrives. Nurses work with families until the baby is two years old.
Who can use this resource? Must be pregnant with your first child and enroll before the end of 28 weeks in your pregnancy. You are income eligible if you are already enrolled in any of the following programs: WIC, FoodShare, Medicaid, or the W-2 program.

About this resource: Prenatal Care Coordination helps women get the support and services she needs to have a healthy baby. You can enroll for this program at anytime within your pregnancy though the sooner, the better. Nurses work with families until the baby is 2 years old.
Who can use this resource? Pregnant individuals may qualify if they are eligible for other programs such as WIC, FoodShare, Medicaid, or the W-2 program.
