About this resource: Access Community Health Centers offer medical, dental, and behavioral health services on a sliding scale. Patients may be asked to pay a minimum at the time of their appointment. Any remaining charges will be billed to the patient.

About this resource: Our Lady of Hope Clinic provides direct primary care to uninsured patients in the Madison area.

Our Lady of Hope Clinic

Additional Information:

Our Lady of Hope Clinic is open Monday-Friday 9am-12pm (closed 12pm-1pm) and then 1pm-4:30pm. 

 

If you are undocumented or do not have health insurance, Our Lady of Hope advises that you:

  1.  Arrive 30 minutes before the clinic opens

    • Either 8:30 am (before they open at 9am) or 12:30pm (before they reopen at 1pm).

  2.  Read instructions on the door and fill out a slip on paper and put it in a box outside the door.

  3.  Wait until doors reopen and wait to see if doctors are able to give you an appointment that day.

 

Our Lady of Hope advises that you bring someone to interpret for you if possible, but they do have one interpreter on site if needed. 

Primary Access for Kids (PAK)

About this resource: PAK is for children who do not qualify for other health insurance. PAK is not an insurance program but allows children to receive care from a primary care provider for free at a designated clinic. Services include annual physical exams, visits to a primary care provider at your assigned clinic, some prescription medications, and transportation assistance.

Who can use this resource? Madison Metropolitan School District kids who do not have health insurance and cannot qualify for BadgerCare or insurance through the Marketplace.
