Public Health Nurse Family Partnership Program & Prenatal Care Coordination Program

 

The goal of the Nurse Family Partnership Nurse Home Visit Program is to improve pregnancy outcomes, child health/development, and the economic self-sufficiency of the family. A free personal nurse would meet with you during your pregnancy and after your baby is born—offering support, health information, help finding community services and so much more! The goal of the Prenatal Care Coordination Nurse Home Visit Program is to work with pregnant women facing challenges to have a healthy birth. Nurses follow families until the infant is 2 months old.  

 

Both of the Nurse Home Visiting Programs are free and voluntary. We request that there is client interest and permission before making a referral. Clients do not need to be a legal resident and interpreter services are available for all clients. For more information and to access a referral form, see here.

Early Childhood Initiative // RISE

Welcome Baby & Beyond // RISE

About this resource: Welcome Baby & Beyond is a free, strengths-based home visiting program for pregnant women and parents or caregivers with children ages 0-5. It builds confidence in parenting skills and helps caregivers understand their child’s development needs from birth to age 5.

Who can use this resource? Parents or primary caregivers that live in Dane County. Individuals must be pregnant at the time of referral or have a child under the age of 5

ParentChild+ // RISE

About this resource: ParentChild+ is an early literacy, strengths-based home visiting program that helps parents and caregivers prepare their children for 4K and school success.

Who can use this resource? Eligible individuals must live in Dane County, children must be 2 years old on or before September 1st. Parent education and income restrictions may apply. Participation is not dependent on citizenship status or languages spokes.
